29 CFR 1917.50(c)(2):Certificates attesting to the required examination of bulk cargo spouts and suckers, together with any portable extensions and rigging or outriggers supporting them vertically, were not made readily available for inspection. a) Ash Grove Cement 3737 N Port Center Way, Portland, Oregon 97217, North terminal, Vessel Skomder Island, On or about 7 June, 2023 and at times prior thereto, employees were exposed to malfunctioning or operation failure conditions of the sucker spout device due to the sucker spout device was not properly OSHA certified. Employer could not produce the required OSHA 71 Certificate for the sucker spout. Resulting in employees exposed to struck by/ crush hazardous conditions resulting in serious injuries or permanent disability.
