1910.147(c)(7)(iii)(B) Additional retraining shall also be conducted whenever a periodic inspection under paragraph (c)(6) of this section reveals, or whenever the employer has reason to believe that there are deviations from or inadequacies in the employee's knowledge or use of the energy control procedures.  a) Toledo Molding & Die, LLC. - Bowling Green, OH: On or about June 8, 2023, the employer did not ensure employees assigned to perform material change over tasks on press 24 surge bin were adequately trained on effective energy isolation steps. Employees performing this new work task, without the benefit of this training were exposed to amputation hazards.

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a)Toledo Molding & Die, LLC. -Bowling Green, OH: On or about June 8, 2023: The employer did not ensure that energy control application steps to control all potential hazardous energy sources were implemented prior to performing cleaning and change over activities. Employees were exposed to amputation injuries in that the press 24 surge bin was not locked out prior to cleaning for product change over.

29 CFR  1910.147(c)(4)(ii):The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this sec:  a) Toledo Molding & Die, LLC. Bowling Green, OH: On or about June 8, 2023, employees were exposed to amputation hazards in that the energy control procedures for the Press 24 surge bin did not identify specific procedural steps for employees to make informed decisions on how to isolate and control the hazardous energy(s) during material change over.
