29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a)  On or about June 7, 2023, at the facility, an energy control program, including specific procedures, employee training and periodic inspections was not developed for maintenance employees performing service and maintenance tasks on equipment such as, but not limited to, the Air Chicago folding machines.

29 CFR  1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  a) On or about June 7, 2023, adjacent to Air Chicago 4, 5, and 6, where the outlet box  powering a fan was damaged, exposing insulated conductor wires.   b) On or about June 7, 2023, at the exhaust fan panel, where a breaker panel had a blank missing, exposing the bus bar of the panel.  c) On or about June 7, 2023, adjacent to Air Chicago 4, 5, and 6, where the conduit was not connected to the outlet box, exposing insulated conductor wires.
