29 CFR 1904.39(a)(2):Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. (a) On or about June 7, 2023 at a jobsite located at 4115 WI-28, Kewaskum, WI 53040, the employer failed to report the work-related incident that resulted in an in-patient hospitalization of the injured employee within 24 hours.

29 CFR 1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. (a) On or about June 7, 2023, at the jobsite located at 4115 WI-28, Kewaskum, WI 53040, employees conducting roofing work on a two story residential home did not receive fall protection training.
