29 CFR 1910.269(a)(2)(iv): The employer did not determine that each employee was complying with all required safety-related work practices required by this section by regular supervision and annual inspections: a) 1187 S Sabinas Dr, Pueblo West, CO 81007, Station #211735 Transformer: On or about June 5th, 2023 and at times prior, employees were exposed to electrical shock hazards from approaching the minimum approach distance of the exposed elbow in Station #211735 Transformer that had not been properly deenergized and/or grounded. The 200 Amp Elbow contacted the employee's bare left hand as the employee was attempting to replace the damaged elbow, resulting in an electrical shock of approximately 7200 volts. San Isabel Electric Association Inc failed to determine that each employee was complying with all required safety related work practices by not providing regular supervision during after-hours jobs.
