29 CFR 1910.178(l)(3)(ii)(I):Powered industrial truck operators did not receive initial training in other unique or potentially hazardous conditions of the workplace that could affect safe operation: (a)Lineage Logistics, LLC dba Lineage Logistics Sioux Falls at 5050 West Foundation Court, Sioux Falls, SD 57107: On or about and at times prior to June 7, 2023, the employer did not ensure that pallet jack operators were trained on specific operations such as U-turns within the warehouse area. This condition exposed employees to crushing, caught-in and pinch hazards between the lift and the warehouse shelving due to employees being allowed to perform U-turns in aisle ways.
