(a) 400 Cox Ferry Flora, MS - On or about June 01, 2023, an employee operating the Peterbilt dump truck did not maintain a 10 feet clearance when the bed was raised from its resting position and made contact with a 13.83 kV, three-phase transmission line, exposing the employees to electrical hazards.
