Employees were exposed to slips and falls. b. On or about November 1, 2023, at the refractory platform, a safe means of access and egress into and out of a ladle was not provided to employees who worked inside a ladle. Employees were exposed to slips and falls. c. On or about November 1, 2023, at a refractory platform, a safe means of access and egress into and out of a tundish was not provided to employees who worked inside a tundish. Employees were exposed to slips and falls.

On or about October 4, 2023, and at times prior thereto, at the refractory platform, the side rails of a portable ladder did not extend 3 feet above the top of the ladle when employees used the ladder to enter and exit the ladle. Employees were exposed to falls from the ladder. b. On or about November 1, 2023, and at times prior thereto, at the refractory platform, the side rails of a portable ladder did not extend 3 feet above the top of the ladle when employees used the ladder to enter and exit the ladle. Employees were exposed to falls from the ladder.
