29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA From 300 or equivalent), was not completed in detail as required by the regulation: Located at Triangle Engineering, Inc. located at 1101 North Redmond Rd Jacksonville, AR 72076: On or about June 8, 2023, the OSHA 300 log for the calendar year 2023 was not completed in the detail required in that: a) On May 25, 2023, Laborer Painter - operating an industrial band saw, made contact with the blade, amputating right thumb. The description of the injury did not include the object/substance that directly injured the employee.
