29 CFR  1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.   A) CA Norris LLC. on or about June 08, 2023 at the address of 712 West 38th Street, Erie, PA: Two employees were constructing steel decking material on the underside of the steel beams over 15-feet above the ground below. Employees were fastening steel decking to the underside of the structure. Employees were not utilizing any form of fall protection while exposed to a fall hazard.

29 CFR  1926.453(b)(2)(v):A harness was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  A) CA Norris LLC. on or about June 08, 2023 at the address of 712 West 38th Street, Erie, PA: One employee operating a Sky Jack SJ45T aerial lift was not attached to the basket while moving decking material roughly 10 feet in the air. The employee was wearing a harness with the lanyard attached to the harness, but not to the basket of the lift.
