29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   a) The Wilson Bohannan Co. located at 621 Buckeye Street, Marion, Ohio, 43302: On or about June 8, 2023, the employer failed to ensure employees were protected from amputation injuries while performing activities such as, but not limited to, set up and change overs activities inside the  Giuliani lock body machine.  The employer failed to ensure that the energy control procedures to control the electrical, pneumatic and hydraulic energy sources were implemented prior to the maintenance and set up tasks.   b) The Wilson Bohannan Co. located at 621 Buckeye Street, Marion, Ohio, 43302: On or about June 8, 2023, the employer failed to ensure employees were protected from amputation injuries while performing activities, such as, but not limited to, set up and changing tooling inside the main door of the Tornos Swiss ST26 machine. The employer failed to ensure that the energy control procedures to control the electrical and pneumatic energy sources were implemented prior to the maintenance and set up tasks.

29 CFR  1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a) Wilson Bohannan Co. located at 621 Buckeye Street, Marion, Ohio: On or about June 8, 2023, the employer failed to ensure periodic audits of the energy control procedures were completed at least annually.  Employees tasked to perform set up and other servicing activities on machines, including but not limited to the Giuliana Body Machine, which require the use of energy control procedures that have not been adequately evaluated for deficiencies.

29 CFR  1910.147(c)(7)(i):The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  a) The Wilson Bohannan Company located in Bucyrus, Ohio: On or about June 8, 2023, the employer failed to ensure that employees who are tasked with set up, servicing or other maintenance activities on equipment such as, but not limited to the Giuliani body machine were adequately trained on the purpose and elements of the safe application and use relating to the isolation and control of hazardous energy sources.  Employees tasked with set up, servicing, and other maintenance activities at or near the pinch points on the aforementioned machine were not specifically trained to lockout the machine to protect against amputation/fracture injuries.
