Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   a) Commercial building located at 414 Eagles Landing Pkwy Dr, back roof area: On or about 05/30/2023, and at times prior, the employer exposed employees to fall hazards of approximately 15-feet, 2-inches to the lower level on a flat roof, in that a fall protection system was not provided as employees were engaged in roofing activities.

Commercial building located at 414 Eagles Landing Pkwy Dr, Stockbridge, GA, back roof area: On or about 05/30/2023, The employer did not designate a competent person to monitor the safety of the other employees while employees  were exposed to a fall hazard of approximately 15-feet, 2-inches while working on a low slope roof near leading edge.

29 CFR  1926.552(a)(4): Hoisting ropes were not installed in accordance with the wire rope manufacturers' recommendations.  a) Commercial construction located at 414 Eagles Landing Pkwy Dr, Stockbridge, GA; back roof area: On or about 05/30/2023, the employer exposed employees to struck-by hazards while operating a TranzSporter TP250, in that the manufactured installed wire rope was replaced by a nylon climbing rope, which is not a proper replacement cable as indicated by the manufacturer of the material hoist equipment.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) Commercial construction located on 414 Eagles Landing Pkwy, back area roof: On or about 05/30/2023, the employer exposed employees to fall hazards approximately 15-feet, 2-inches to lower ground level, in that a ladder with extended side rails at least 3-feet past the roof eave was not provided to employees accessing the plate and ground levels.
