29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) 25 E Allen St, Philadelphia, PA, Philadelphia Distilling - Warehouse area/Behind distilling area: On or about June 9, 2023, the employer did not ensure each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation. Employees operating powered industrial trucks (PITs) but not limited to, Yale Forklift (Serial: B807N03307V), were not trained in the safe operation of powered industrial trucks, exposing employees to struck-by hazards. Training shall consist of a combination of formal instruction, practical training, and evaluation of the operator's performance in the workplace.
