29 CFR  1910.179(j)(2)(iii):Hooks with deformation or cracks. Visual inspection daily; monthly inspection with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected. For hooks with cracks or having more than 15 percent in excess of normal throat opening or more than 10�� twist from the plane of the unbent hook refer to paragraph (l)(3)(iii)(a) of this section.  On or about June 8, 2023, employees engaged in the metal fabrication were exposed to struck by hazards due to employees using a hook for lifting that was damaged in the following way: a. Main Hoist-Hoist -1 number 51 Hook chain hook missing gate b. No visual inspection or certification was obtained  for Main Hoist-Hoist -1 number 51 Hook  c. the only chain hook combination had a tag that was not legible

29 CFR  1910.179(j)(2)(iv):Hoist chains, including end connections, for excessive wear, twist, distorted links interfering with proper function, or stretch beyond manufacturer's recommendations. Visual inspection daily; monthly inspection with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected.  On or about June 08, 2023 employees engaged in the metal fabrication were exposed to struck by hazards due to the chains not being inspected.

29 CFR  1910.184(h)(1):Sling use. Employers must use natural and synthetic fiber-rope slings that have permanently affixed and legible identification markings stating the rated capacity for the type(s) of hitch(es) used and the angle upon which it is based, type of fiber material, and the number of legs if more than one.  On or about June 8, 2023, employees engaged in the metal fabrication were exposed to struck by hazards due to employees using a sling that did not have the required marking and  was frayed.

29 CFR  1910.215(a)(4): Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion.   On or about June 8, 2023, employees engaged in the metal fabrication were exposed to caught between hazards due to using a bench grinder to grind tungsten without the work rests.
