29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about 6/9/2023, and at times prior thereto, employees conducted roofing operations at the ground level and were not protected from falling roofing materials being removed from the workplace one-story building roof. This condition exposed employee to struck by hazards 8'4" above their heads.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems. On or about 6/9/2023, and at times prior thereto, employees conducted roofing operations at heights of greater than 6 feet above a lower level did not have a fall protection system in place. This exposed employees to a fall of about 8'4" from a 10:12 pitched roof.
