29 CFR  1926.404(f)(1)(iv)(A): AC system of 50 volts to 1000 volts was not grounded. The system could be so grounded that the maximum voltage to ground on the ungrounded conductors would not exceed 150 volts.  On or about June 09, 2023, one of the forced air hepa filtration units being used in the asbestos abatement process was not grounded.  This condition exposed employees to electrical hazards.

29 CFR  1926.404(b)(1)(iii)(C):The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.  On or about June 09, 2023 flexible cord sets being used were not inspected before use for external defects  such as insulation damage and missing pins. 29 CFR  1910.134(g)(1)(i)(A):Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function.   On or about June 09, 2023, employees with facial hair were allowed to wear air purifying respirators while working on a controlled asbestos abatement /demolition zone were fibers sampled on blank cassettes had more than 2 fibers per cubic centimeter.  This condition exposed employees to health hazards such as illnesses that can lead to asbestosis or lung cancer.

29 CFR  1926.1101(g)(1)(iii):The employer did not use work practices in all operations covered by 29 CFR 1926.1101, regardless of the levels of exposure, in the form of prompt clean-up and disposal of wastes and debris contaminated with asbestos, in leak-tight containers:  On or about June 09, 2023 two employees were observed removing debris that contained material with 10 % of asbestos chrysolite with a shovel and disposing of it on a bobcat loader that was not leak-tight. This condition exposed employees to asbestos health hazards.
