29 CFR 1910.147(c)(5)(i):Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy source: a) 3140 Bordentown Ave, Old Bridge, NJ -Two employees were working dismantling a mobile manual gantry crane. One employee released the nut from the base and the support leg from the manually operated gantry crane. As the nut was removed, the support leg slightly shifted clockwise pinching the tip of the employee?s finger causing an ?amputation to the distal tuft of the left fourth finger, the very tip of the finger. Incident occurred on 5/31/23.

29 CFR 1910.147(c)(7)(i):The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: a) 3140 Bordentown Ave, Old Bridge, NJ 08859 -Equipment Yard - The employer did not ensure employees engaged in the dissembling of a gantry crane were provided adequate training to ensure the isolation of unexpected release of stored energy. The cross beam section of the crane shifted, while employees were removing the bolts on the lower base plate, and caused the injury. Incident occurred on 5/31/23.
