29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Top of roof - On or about June 12, 2023, the employer provided fall protection for his employees but failed to ensure the employees working on the steep roof applying roof sheeting were attached to the lanyards exposing them to fall hazards.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) West and East side of house - On or about June 12, 2023, the employer allowed his employees to use the aluminum extension ladders to gain access to the roof that did not extend 3 feet above exposing them to fall hazards.
