29 CFR  1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:   (a)Orion Food Systems, L.L.C. dba OLM Food Solutions at 2404 Marietta Place, Sioux Falls, SD 57107: On or about and at times prior to June 5, 2023, the employer failed to ensure employees were protected from energized machine components by using existing lockout/tagout procedures while servicing the Admatic dough baller which required the machine to be turned off or shut down to remove excess dough accumulation. On June 5, 2023, an employee was provided an interlock bypass key that disabled the safety feature and avoided the shutdown of the machine to remove excess dough. As a result, an employee was exposed to energized machine components and sustained a fingertip amputation. This condition exposed employees to caught-in and amputation hazard.

This condition exposed employees to crush-by hazards associated with unexpected forklift performance.
