This was demonstrated on or about and at times prior to June 9, 2023, when employees working in excavations were not protected from cave-in hazards while installing sanitary sewer lines, in that the excavation was measured greater than five feet deep and lacked a protective system, such as appropriate sloping, benching, or shield system.   This condition exposes employees to cave-in and struck-by hazards when performing work in the unprotected excavation.  SiteWorks Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1615990, citation number 1, item number 1a and was affirmed as a final order on June 12, 2023, with respect to a workplace located at the intersection of 41st Street and Valleyview Road, Sioux Falls, SD 57106.  SiteWorks Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1578341, citation number 2, item number 1a and was affirmed as a final order on July 26, 2022, with respect to a workplace located at the North Oaks Avenue Addition, Harford Industrial Park Sewer Extension Project, Hartford, SD 57033.
