29 CFR  1960.66(b):Federal agency injury and illness recording and reporting shall comply with the requirements under 29 CFR 1904, subparts C, D, E, and G, except that the definition of "establishment" found in 29 CFR 1960.2(h) will remain applicable to Federal agencies.  29 CFR  1904.40(a) reference: The employer had not provided an authorized government representative the records within the four business hours.  JOHN F KENNEDY DAY SCHOOL: On or about June 12, 2023, the employer failed to provide copies of the requested injury and illness records for calendar years 2020, 2021, 2022 and 2023 to an authorized OSHA representative within four business hours after records were requested.  The Bureau of Indian Education was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1960.66(b), which was contained in OSHA inspection number 1502674, citation number 2, item number 2, issued on January 19, 2021, which was affirmed as a final order on March 12, 2012 with respect to a workplace located at Navajo Rte. 27, 18 mi N. of Ganado, Ganado, AZ, 86505.

29 CFR 1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  Front of gym: Employees who were required to wear fall protection while using a Snorkel boom aerial lift while painting had not been trained on fall protection systems, exposing them to hazards from falls.
