JOHN F KENNEDY DAY SCHOOL: On or about June 12, 2023, the employer failed to provide copies of the requested injury and illness records for calendar years 2020, 2021, 2022 and 2023 to an authorized OSHA representative within four business hours after records were requested.  The Bureau of Indian Education was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1960.66(b), which was contained in OSHA inspection number 1502674, citation number 2, item number 2, issued on January 19, 2021, which was affirmed as a final order on March 12, 2012 with respect to a workplace located at Navajo Rte. 27, 18 mi N. of Ganado, Ganado, AZ, 86505.
