(Construction reference 29 CFR 1926.602(d)):  a) Exterior of Building - An employee was exposed to fall and struck by hazards as the Genie Forklift was operated by an employee who was neither trained nor evaluated in the safe operation of the equipment, on or about 6/12/2023.

29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used for access:   a-(Deck) 257 Wanaque Ave, Pompton Lakes NJ 07442:  An employee engaged in deck beam installation activities was observed climbing the scaffold members and exposed to fall hazards of approximately 18ft to the concrete deck below as a means of scaffold access was not utilized. Condition observed on or about 06/12/2023.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a-(Deck) 257 Wanaque Ave, Pompton Lakes NJ 07442:  Employees engaged in deck beam installation activities were positioned on a at an unprotected deck edge exposed to fall hazards of approximately 18-20ft to the asphalt below in which a means of fall protection was not being utilized. Condition observed on or about 06/12/2023.
