The scaffold's screen was not attached to the scaffold's walking and working surface its entire length, the scaffold's work rest was not connected to its supporting bracket, the poles of the scaffold were not braced properly and the scaffold's poles were not plumb.  Condition observed on or about 06/13/23.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
