29 CFR 1926.1053(b)(1): Portable ladders are used for access to an upper landing surface and the ladder side rails do not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: On or about 04/14/2023, the employer did not ensure portable ladders were constructed and placed so three (3) feet or more was extended above the upper landing. Workers were delivering residential construction materials to the roof and were exposed to falling approximately twenty (20) feet while using an extension ladder to ascend/descend from the ground level to the roof.
