29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: A. Ironer #2: On, or about, June 7, 2023, an employee suffered the partial amputation of two fingers while performing preventative maintenance tasks on the machine. The machine was not locked out in accordance with it's lockout procedure. B. Tri-Edge Maxx Feeder #1: On, or about, June 4, 2023, the line side power feeding the electrical control box was not de-energized while the box was opened for cleaning with compressed air.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: Facility: On, or about, June 7, 2023, lockout/tagout training provided to maintenance technicians was not adequate in that authorized employees were not aware that machinery and equipment was required to be isolated from energy sources and locked out during preventative maintenance tasks.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items (A) through (D) of this section: Tri-Edge Maxx Feeder #1: On, or about, June 4, 2023, the specific lockout procedure developed for the machine was not adequate in that it did not address de-energizing the line side power feeding the electrical control box during cleaning operations. Two other isolation points were available to be used for the task, a cord and plug buss drop cord and buss mounted disconnect.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: Tri-Edge Maxx Feeder #1: On, or about, June 4, 2023, employees who performed weekly preventative maintenance on the machine did not affix their own locks and tags to each isolating device. Work was performed with the equipment locked out by the Maintenance Supervisor.
