29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910: A. Building 130, Conference Room: On, or about, June 9, 2023, sufficient working space was not provided for Panel RP1B, a 120/208 volt Culter-Hammer panelboard. A storage unit, moving dolly and fire extinguisher were located in the working space. B. Building 40, Tool Room: On, or about, June 9, 2023, sufficient working space was not provided for an Eaton 480 volt fused disconnect that was located on the wall behind a Bridgeport milling machine, serial number J262374.

29 CFR 1910.305(g)(1)(iv)(E): Flexible cords and cables were used where concealed behind building walls, ceilings, or floors: Building 130, ECR 2: On, or about, June 9, 2023, Type SOOW flexible cord was used to energize the four extruding lines located in the room. The flexible cord was run through holes in the suspended ceiling tiles and through the concealed space above the ceiling.

The opening at position 2 was covered with electrical tape and the opening at position 6 was not covered.
