Theut's Flower Barn Inc. dba RJ's Greenhouse located at 909 Lemoyne Rd, Northwood, Ohio 43619 :  On or about April 6, 2023, The employer did not ensure employees were protected from falls while replacing polycarbonate panels on the roof of the Fern Greenhouse. The employees were exposed to a fall distance of approximately 9 feet to the surface below.

29 CFR  1910.147(c)(7)(iv): The employer did not certified that employee training had been accomplished:  a. Theut's Flower Barn Inc. dba RJ's Greenhouse located at 909 Lemoyne Rd, Northwood, Ohio 43619 :  On or about June 14, 2023, the employer failed to certify that employees were trained in the control of hazardous energy prior to the employees performance of service and maintenance activities, on equipment such as but not limited to: motors and ventilation fans. Without the benefit of employer provided training, employees tasked with maintenance of drive belts on the aforementioned equipment were exposed to caught in hazards.

Theut's Flower Barn Inc. dba RJ's Greenhouse located at 909 Lemoyne Rd, Northwood, Ohio 43619 :  On or about June 14, 2023, The employer did not develop and implement the elements of a written hazard communication program for employees exposed to hazardous chemicals such as, but not limited to thiophanate methyl and propylene glycol which are contained in the product(s) Organosilicone Surfactant, and 3336F Fungicide. Employees tasked with handling these material were not provided with the information necessary to protect themselves from eye, respiratory, and dermal hazards.
