29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) Storage Condominiums Inc. On or about June 13, 2023 at the address of 3750 Clean St., Port Clinton, OH 43452. The employer failed to train employees in the recognition and avoidance of hazards commonly seen in the construction industry. Employees were not benefited by training to aid in the recognition of hazards such as but not limited to falls during the performance of their assigned work tasks.

29 CFR  1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a)	Storage Condominiums Inc. On or about June 13, 2023 at the address of 3750 Clean St., Port Clinton, OH 43452. The employer did not ensure employees were adequately protected from falls when utilizing a scissor lift (model: JLG Commander 1-006A) to perform roofing work activities. Employees had bypass the guardrail system by climbing on the platform's mid and top rails as a way of gaining access to the roof level.  Employees were exposed to falls of 19 feet or greater to the ground below.

29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a)	Storage Condominiums Inc. On or about June 13, 2023 at the address of 3750 Clean St., Port Clinton, OH 43452. The employer did not ensure employees were protected from fall hazards while working atop a low sloped commercial structure. Employees installing roofing underlayment  paper atop the roof structure with a 3-12 pitch were exposed to falls of 19 feet to the ground below.
