29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: At a worksite located at 2201 Roark Valley Rd, Branson, MO 65616: a) Employees were exposed to unexpected start-up or release of stored energy. This was demonstrated on or about June 14, 2023, and at times prior to, when employees where performing servicing and maintenance activities, including but not limited to the Chicago Dryer Co. (Cros Star CT36 Gas/Serial #64680) iron and folding machine, because the employer did not clearly and specifically outline the machine specific steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. b) Employees were exposed to unexpected start-up or release of stored energy. This was demonstrated on or about June 14, 2023, and at times prior to, when employees where performing servicing and maintenance activities, including but not limited to the Chicago Dryer Co. (Air Chicago Elite/Serial #64679) towel folding machine, because the employer did not clearly and specifically outline the machine specific steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.

29 CFR 1910.304(b)(3)(ii)(C)(3): Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug, was not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage: At a worksite located at 2201 Roark Valley Rd, Branson, MO 65616: Employees were exposed to electrical shock hazards. This was demonstrated on or about June 14, 2023, and at times prior to, when a portable floor fan (serial # 2477842/model #HVP-30E) was connected in series to a cord and cable (extension cord), and the plug end for the portable fan was missing a ground pin. The employer failed to ensure that each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug, was not visually inspected before each day's use for external defects, such as deformed or missing pins.
