1910.147(c)(7)(i)(A) Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about June 9, 2023, at a job site located at 14069 Liv 261 Chillicothe, Missouri, the employer exposed employees to a crush and struck-by hazard, in that employees did not receive retraining addressing hazards posed by hydraulic drift of raised hydraulic arms of a front end loader while working in near proximity to a front end loader.
