29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Location: Quan's Kitchen Restaurant- 30 Chauncy Street- Mansfield, MA On or about May 4, 2023, an employee suffered an injury resulting in the amputation of two fingers while attempting to retrieve a plastic spatula from a mixer/grinder machine which was not guarded to prevent his hand from contacting the mixing arm assembly.

Location: Quan's Kitchen Restaurant- 30 Chauncy Street- Mansfield, MA On or about May 4, 2023, an employee lost his index and middle finger on his right hand while working at Quan's Kitchen. The employer did not report this work-related amputation to OSHA.
