29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) Imasen Bucyrus Technology, Inc. located at 260 Crossroads Blvd. Bucyrus, Ohio, 44820: On or about June 7, 2023, the employer failed to ensure employees were protected from amputation injuries while performing activities such as, but not limited to, attempting to clear a seat that was jammed in the Quiet Rooms (QA1 and QA2) of the automated Powerline 1 located at the South End of the building. The employer failed to ensure that the energy control procedures were utilized to control the electrical, pneumatic, and hydraulic energy sources prior to the employee performing this task.
