Employees in the 750 and 800 warehouse area were exposed to a serious struck by hazard from the Toyota Forklift , model 058025, serial number 55B025-117, known to the company as forklift 130, that was in operation, without the rubber cover on the gas pedal.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: At a worksite located at 1600 N State Route 291, Suite 750, Independence, Missouri. Employees were exposed to Electrical Shock injuries, in that the electrical cord on the RIDGID brand MS1250 12in, Part Number-SP6449 Miter saw in use did not have a ground pin to ensure the path to ground was permanent, continuous and effective.
