29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) On June 15, 2023, at 13947 Landstar Blvd, Orlando, FL 32824 ; employees were exposed employees to 8-foot fall hazards to the next lower level, in that, ladders or safe access was not provided.

29 CFR 1926.452(c)(3): Frames and panels were not joined together vertically by coupling or stacking pins or equivalent means: a) On June 15, 2023, at 13947 Landstar Blvd, Orlando, FL 32824; the employer exposed employees to fall hazards of approximately 8 feet to the next lower level. The employer did not ensure that all stacking pins and couplings were secured to prevent the cross braces from separating from the scaffold.
