Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about June 16, 2023, the employer did not ensure appropriate fall protection was implemented for employees performing commercial roofing operations on a flat roof that was greater than 50 feet wide. The employees were exposed to fall hazards of approximately ten (10) feet.
