Fall protection was not utilized.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At the jobsite located at 4200 W 158th St. Overland Park, Kansas, employees engaged in residential framing activities were exposed to fall hazards when accessing an extension ladder in that the ladder did not extent at least 3 feet above the landing surface. Werner extension ladder utilized on the side of the residential building extended 1 foot beyond the top of the wall at a height of 11 feet.
