29 CFR  1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a.)  General Partitions Manufacturing Corp.;   2424 W. 23rd Street, Erie, PA - On or about 6/20/23, and at times prior thereto,  the company had not performed periodic inspections of the energy control/lockout tagout procedures. Employees conducted servicing, maintenance and repair on equipment that included, but was not limited to, the Edge Bander.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

General Partitions Manufacturing Corp; 2424 W. 23rd Street, Erie, PA - On or about 6/20/23, and at times prior thereto, the company had not provided lockout/tagout training on the employer's energy control/lockout tagout procedures and program for authorized employees. Employees conducted servicing and maintenance on equipment, including, but not limited to, the Edge Bander.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

General Partitions Manufacturing Corp;  2424 W. 23rd Street, Erie, PA - On or about 6/20/23, and at times prior thereto, the company had not provided lockout/tagout training on the employer's energy control/lockout tagout procedures and program for affected employees.   Employees performed servicing and maintenance on equipment including, but not limited to, Edge Bander and conveyor.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck   a.) General Partitions Manufacturing Corp.; 2424 W. 23rd Street- On or about 6/20/23, and at times prior thereto, the company was permitting the employees to operate forklifts without being trained on the forklifts. Employees used forklifts to move stacks of materials in the facility.  The propane forklifts included, but were not limited to, Hyster, CAT, and Yale.    ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

29 CFR  1910.303(b)(1)(v):Safety of electrical equipment was not determined by considering heating effects under all conditions of use.  a.)  General Partitions Manufacturing Corp.-; 2424 W. 233rd Street, Erie, PA  -  On or about 6/20/23, and at times prior thereto, The Edge Bander computer control box was overheating due to the fan inside the box not working.  The box was propped open to keep the box cool while the Edge Bander was in operation.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
