29 CFR  1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders or portable tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:  (a) 1129 Harrisburg Pike, Carlisle, PA 17013; shop floor - On or about June 21, 2023,  the employer did not ensure that gas cylinders used for welding activities,  were secured to prevent them from being knocked over.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.253(b)(4)(iii):Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating:  (a) 1129 Harrisburg Pike, Carlisle, PA 17013; directly outside the shop roll-up door - On or about June 21, 2023, the employer did not ensure that the gas cylinders containing combustible gases were kept separate from gas cylinders containing oxygen by at least 20 feet.  No abatement certification or documentation required.

29 CFR  1910.243(c)(4):The angular exposure on the grinding wheel periphery and sides for safety guard(s) used on portable grinding machine(s) exceeded 180 degrees:  (a) 1129 Harrisburg Pike, Carlisle, PA 17013; shop floor - On or about June 21, 2023, employees operated Dewalt and Milwaukee hand held angle grinders that did not have the safety guards in place.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
