29 CFR 1926.404(b)(1)(i):  The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:  a) On June 20, 2023  Employees were provided with electrical tools and equipment connected by cord and plug without the use of an assured equipment grounding conductor program or ground fault circuit interrupters (GFCI's).  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  On June 20, 2023, employees on the upper main section of the house were exposed to fall hazards of over 25 feet while maneuvering material across a pitched roof surface.        b)  On June 20, 2023, employees on the the lower east section of the house were performing roofing work were exposed to fall hazards of approximately 16  feet.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.552(a)(1):  The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators:    a)  On June 20, 2023, employees were exposed to fall hazards while using a material hoist as a ladder to access the roof.
