1926.20(b)(2) Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. 29 CFR 1926.250(b)(1):Material inside a building(s) under construction was stored within 10 feet of an exterior wall(s) which did not extend above the top of the material being stored LOCATION: From the mezzanine to the floor below in and outside of the building on the construction site at 101 Hampden St. On or about June 21st employees were exposed to struck-by injuries when construction materials and scaffolding were stored within 10 feet of an exterior wall(s) which did not extend above the top of the material being stored.

29 CFR 1926.502(j)(1):Toeboards were not erected along the edge of the overhead walking/working surface for a distance sufficient to protect employees below. LOCATION: various locations from mezzanine level to ground floor below on the jobsite located at 101 Hampden St. On or about June 21st, 2023 the employees on the working/walking floor below the mezzanine were exposed to injury from dropped tools and materials when toeboards were not erected along the edge of the overhead.
