1910.176(b): Storage of material created a hazard: (a) On or about June 21, 2023, at the permanent job site located at 3607 Ronald Reagan Blvd. in Johnstown, CO 80534: Employees were required to store, add and pick materials from an employer provided multi-level storage rack system. This storage rack system is made up of steel structural members in the form of a frame, post and beams to form a bay. The rack system did not have permanent plaque(s) stating the maximum permissible load, and the maximum load per bay as required by the American National Standard Institute (ANSI). This condition exposed employees to overhead struck-by hazards. Abatement Note: Among other feasible and acceptable means of abatement: to abate these hazards comply with the ANSI MH61.1:2012, pg. 2, section 1.4.2, Plaque.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: (a) On or about June 21, 2023, at the permanent job site located at 3607 Ronald Reagan Blvd. in Johnstown, CO 80534: Employees were exposed to fire and explosion hazards. Employees engaged in powered industrial truck operations near compressed gas cylinders stored near travel paths, were exposed to serious hazards in that the employer did not store compressed gas cylinders in accordance with Compressed Gas Association Pamphlet P-1-1965.
