29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: On or about June 20, 2023, the employer did not ensure that employees were trained as authorized employees prior to allowing them to engage in lockout/tagout activities and beginning contracted work to modify the (TimkenSteel) Faircrest Steel Plant Vertical Caster West Tundish Car. Employees were not trained on the recognition of hazardous energy sources and the methods and means necessary to control them. These employees were exposed to crushing/caught-between hazards that resulted in a serious injury/hospitalization.

29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and de-energization of the machine or equipment had been accomplished. On or about June 20, 2023, the employer failed to ensure employees working as outside contractors and performing service and maintenance activities by modifying the (TimkenSteel) Faircrest Steel Plant Vertical Caster West Tundish Car, verified that hazardous energy had been isolated and deenergized by; turning off or shutting down the scissors lift machine attached to the West Tundish Car platform, affixing lockout device(s), and verifying de-energization and isolation of the scissors lift machine had been accomplished, prior to modifying the West Tundish Car.
