On or about June 20, 2023, the employer failed to ensure employees working as outside contractors and performing service and maintenance activities by modifying the (TimkenSteel) Faircrest Steel Plant Vertical Caster West Tundish Car, verified that hazardous energy had been isolated and deenergized by; turning off or shutting down the scissors lift machine attached to the West Tundish Car platform, affixing lockout device(s),  and verifying de-energization and isolation of the scissors lift machine had been accomplished, prior to modifying the West Tundish Car.
