29 CFR  1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as follows:   a) Job site, 200 Park Manor Drive: On or about June 21, 2023, during masonry operations, the employer did not ensure that an employee was working on a fully planked work platform in that he worked on a level with only one approximately 9 inch wide plank.

29 CFR  1926.451(c)(1)(iii): Ties, guys, braces, or outriggers were not used to prevent the tipping of supported scaffolds in all circumstances where an eccentric load, such as a cantilevered work platform, was applied or transmitted to the scaffold:    a) Job site, 200 Park Manor Drive: On or about June 21, 2023, during masonry operations, the employer did not ensure that employees worked on a scaffold that was safe from eccentric load forces in that a wooden scaffold was placed on the building roof at one end and on a metal supporting scaffold at the other end, with no means of restraint in use to prevent the metal supporting scaffold from tipping over from the eccentric load posed by the wooden scaffold.

29 CFR  1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:    a) Job site, 200 Park Manor Drive: On or about June 21, 2023, during masonry operations, the employer did not ensure that a competent person oversaw and approved of the scaffolding set up. Deficiencies noted during the inspection included, but were not limited to, the lack of  appropriate fall protection and cross bracing, working levels not being fully planked and failing to ensure that scaffolding was protected against a tipover hazard from an eccentric load being placed on it.
