29 CFR  1926.20(b)(2): The employer did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer:   a) Job site, 200 Park Manor Drive, Pittsburgh, PA: On or about June 21, 2023, during construction activities, the employer did not ensure that  a competent person was conducting frequent and regular inspections of the job site to identify and correct unsafe working conditions. Deficiencies noted during the inspection included, but were not limited to, allowing subcontractor staff to work on scaffolding that had inadequate fall protection and cross bracing, permitting work to be done on scaffold working levels that were not fully planked and failing to ensure that that scaffolds with eccentric loads put on them were secured in an appropriate manner to prevent tip-overs or collapses.
