On a 5:12 picthed roof of a temporary residential new construction worksite, located at 9104 Joannes Way - Lot 27: On or about June 21, 2023, the employer exposed an employee to fall hazards ranging from 8 feet to 16-feet 8 inches, in that fall protection was not utilized by the employee while installing underlayment paper.

On a temporary residential work site located at 9104 Joannes Way - Lot 27, Jacksonville, FL 32222: On or about June 21, 2023, the employer exposed an employee to fall hazards, in that the ladder's side rails did not extend at least 3 feet above the roof. The employee was being exposed to fall hazards.
