OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: On or about June 18, 2023: Employees engaged in lactic acid production field activities at the Cargill CLA300 building located at 650 Industrial Park Drive, Blair, Nebraska were exposed to chemical hazards after the employer failed to ensure the maintenance contractor torqued the flowmeter flange bolts to required specifications, contributing to the release of pressurized 4% sodium hydroxide solution in the form of a mist that hospitalized three subcontractor employees and required evacuation of exposed employees from the entire building. FEASIBLE AND ACCEPTABLE ABATEMENT METHODS TO CORRECT THIS HAZARD INCLUDE BUT ARE NOT LIMITED TO: 1) Ensure flange bolting is torqued to the predetermined value after maintenance work is finished. 2) Ensure escaped fluids from flange joints during possible piping failures can be safely confined.
