29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).    (a) On June 20, 2023, an employee was exposed to falls of up to approximately 18 feet while installing sheet metal roofing without fall protection.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) On June 20, 2023, and at times prior, employees performed roofing activities from heights up to approximately 22 feet to the ground on an 8 : 12 sloped roof.  The employer had not verified or certified that employees had received adequate training in the hazards associated with working from heights or how to mitigate them.

29 CFR  1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface, the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     a) On June 20, 2023, and at times prior, employees accessed the with a ladder that extended approximately 2.5 feet above the upper landing surface of the roof.
