29 CFR 1960.8(a): The head of each agency shall furnish to each employee employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm. OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm inn that employees were exposed to fire hazards. a) U.S. Coast Guard Base San Juan, West of Building 110, Near Temporary Hazardous and Universal Waste Storage Area Secondary Containment. On or about June 22nd, 2023, a fire hydrant was obstructed by heavy chains, drums, a plastic pallet and a tow-truck trailer with a pin box in front of approximately 20 to 25 feet in length . This action can delay Fire Department Emergency Personnel response and access to the fire hydrant and exposing employees to fire hazards considering this is the closest hydrant to the Building 110 Shops. A feasible and acceptable methods to correct this hazardous condition would be: 1. Clear the path of access to the fire hydrant. 2. Adhere to and maintain 36 inches of clear space around the circumference of fire hydrants and a clear space of not less than 60 in. (1524 mm) shall be provided in front of each hydrant connection having a diameter greater than 21?2 inches, in accordance with the Consensus Standard NFPA 1, 18.5.7.1 and 18.5.7.2, Clearance around fire hydrants.

29 CFR 1910.157(c)(1):Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: a) On or about June 22nd, 2023, at the US Coast Guard Base San Juan, building 110, Air Conditioning Shop, a fire extinguisher (10-pounds, dry chemical) not in its designated place, instead it was laying on a corner of the room, exposing employees to fire hazards. b) On or about June 22nd, 2023, at the US Coast Guard Base San Juan, building 11, Hazardous Materials and Waste Storage Area, a fire extinguisher, dry-chemical, model B456 was obstructed by a large white plastic box, not allowing for easy access and exposing employees to fire hazards. NOTE: Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903 (c) and (d) respectively.

NOTE: Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer prior to citation issuance.

29 CFR 1910.101(b):The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: a) On or about June 22nd, 2023, at Building 110, Gases Storage Area, six MAPP flammable gas cylinders were not stored in a secured area to avoid them from being struck or damaged, exposing employees to fire and explosion hazards. b) On or about June 22nd, 2023, at Building 110, Gases Storage Area, six liquefied petroleum gas (LPG) tanks with a capacity of 15.2 pounds each, for forklifts were not secured with chains inside the cage to avoid them from falling and getting damaged, exposing employees to explosion hazards. NOTE: Certification and documentation of abatement must be submitted for this citation item as required by CFR 1903.19(c) and (d), respectively.
