Building #2, on or at times prior to June 20, 2023, while accessing the roof through a hatch, and working in close proximity to the opening, subcontractor employees were exposed to falling through the hatch which was left open and unsecured.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a) At 7954 Jacks Way, Horace, ND 58047. Building #2, on or at times prior to June 20, 2023, subcontractor employees engaged in roofing operations on the 4:12 pitch roof of a commercial building were exposed to fall hazards of greater than 20 feet.

Building #2, on or at times prior to June 20, 2023, the employer did not ensure the Little Giant extension ladder used to  access from the first floor to the second first floor extended at least three feet above the upper landing surface.
