29 CFR  1926.405(g)(2)(iii): The repair splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used:  (a) 2nd floor, North west room, on wall to left of door: On or about 21 June 2023, and at times prior, the employer exposed employees to fire and electrical shock hazards in that a Southwire SJTW Plastic Guard Sting Light was spliced together and covered with electrical tape after being completely severed.  (b) 1st floor, North west room, on floor next to doorway on left: On or about 21 June 2023, and at times prior, the employer exposed employees to fire and electrical shock hazards in that a Southwire SJTW Plastic Guard Sting Light was severed and the end repaired incorrectly with electrical tape.

29 CFR  1926.501(c)(3): The employer did not barricade the area to which objects could fall, did not prohibit employees from entering the barricaded area, and did not keep objects that could fall far enough away from the edge of a higher level so that the objects would not go over the edge if they were accidentally displaced.  (a) 2nd floor, North-West room, window: On or about June 21, 2023, and at times prior, the employer exposed employees to possible death or severe head injuries, in that, a Rigid R86001 18V 1/2 Drill/Driver with battery and bit installed (3.79lbs), 16oz claw hammer, 12 inch steel pry bar, aluminum square, shims, Philips screwdriver, and a box of 2 1/2 inch nails, were left sitting unsecured on the sill of an open window located directly above the employee break/sitting area where individuals were sitting without hard hats.

29 CFR  1926.150(a)(2): Access to all available firefighting equipment was not maintained at all times:  (a) Bldg. 20, bottom landing of interior stairwell:  On or about June 21st 2023, and at times prior, the employer exposed employees to fire related hazards, in that a mounted, portable fire extinguisher was not readily available for use because it was being used as a hanger for a safety face shield.
